Who is this for?

This package is intended for individuals and organizations interested in launching Recuperative Care services in Minnesota. These services are geared toward assisting individuals who are homeless and in need of housing while recovering from a medical condition, helping to prevent hospital readmissions. The aim of recuperative care is to support patients on their path to recovery in a nurturing setting. It helps patients regain stability, encouraging healing while they transition back to everyday life. This model serves as a bridge between discharge from a medical facility and the return to independent living or long-term care.

What's Included?

Recuperative care services are available to eligible MHCP (Minnesota Health Care Programs) members who are homeless or unable to recover from illness in a stable environment. These services are available when hospitalization is not necessary, but some level of medical oversight and recovery support is still needed. The current state reimbursement is $300 per day for qualifying recipients. A referral from a medical provider is required to begin services.

How Much Does It Cost?

Comprehensive package: $4,500 - Includes everything from provider enrollment to core policies. Staffing services are not included. Note: This does not include the $730 DHS provider enrollment fee or any insurance credentialing fees.

What Do You Need to Start?

  • 1 A residential setting to operate your program - there are currently no limits on size or zoning.
  • 2 A staffing plan that includes a licensed medical provider or a partnership with a provider to oversee care. This includes submission of the appropriate acknowledgment forms to DHS.
  • 3 Professionals such as RNs, LPNs, case managers, or social workers may provide care after approval. Community Health Workers must enroll individually with MHCP.

Timeline Overview

  • Onboarding (1-2 days): You'll receive the onboarding packet within 2 business days of your consultation.
  • Questionnaire (1-2 days): We gather key information through a client questionnaire.
  • Application Submission (1-2 weeks): We complete and submit your application to DHS.
  • DHS Review (30 days): DHS typically reviews applications within 30 days.
  • Resubmission (if needed): Additional submissions may be required.
